Be sure to verify your healthcare provider's participation in the PHCS Network or
The MultiPlan Network before receiving treatment. Your benefit plan may not offer access
to all providers in our network. In addition, hospital-based physicians may not be covered
under your health care plan even though the hospital is. If you receive services that are
not covered by your plan, you will be responsible for payment to the provider. For
information about specific services, consult your benefits booklet or other health care plan
description.
